I. General Provisions

These "Terms of Use for the Contact Form by Users," hereinafter referred to as the Terms, set out the rules for Users' free-of-charge use of the Contact Form available on the website www.flymedica.pl, for the purpose of obtaining answers to their question(s) and obtaining qualification for a procedure within the subject matter it covers.

II. Definitions

Terms used in these Terms shall mean:

1. Contact Form – the form in which Users of the website www.flymedica.pl provide the information and data necessary to perform the service / process the submitted enquiry.
2. User – a person using the Contact Form to the extent provided for in these Terms.
3. Administrator – Flymedica Sp. z o.o., with its registered office in Poznań at ul. Dąbrowskiego 77a, 60-529, email address: kontakt@flymedica.pl, phone: +48 799 370 038.

III. Rules of User Access to the Contact Form

1. Access to the Contact Form is not conditional on the User completing any formalities; in particular, such access does not require the User to register or log in.
2. The Administrator provides Users with free access to the Contact Form for an indefinite period.
3. The Contact Form is intended solely for correspondence related to the Administrator's business activity, within the subject matter it specifies.

IV. Terms of Use of Flymedica's Contact Form and User Obligations

1. In order to use the Contact Form, the User performs the following actions:
   1. completing the Contact Form by providing the following mandatory data and information: first name, last name, email address, phone number, photos, and the content of the message.
   2. confirming that they have read the Privacy Policy and these Terms, and that they accept all of their provisions,
   3. confirming that they have been informed that the administrator of the personal data provided is Flymedica Sp. z o.o.
   4. confirming that they consent to the processing of their personal data for the purpose of preparing and providing a response to the question(s) asked, and to the deletion and anonymization of such data in accordance with the provisions set out in the Privacy Policy,
   5. confirming the wish to be contacted by clicking the "Send" link contained in the Contact Form.
2. The User is obliged to use the Contact Form in accordance with the law, the provisions of these Terms, and good practice.
3. A User using the contact form is aware that the information provided in it should not contain sensitive/confidential data.
4. The User undertakes not to provide false personal data, not to provide the personal data of another person without that person's permission, and not to provide an email address that is not their own. The User bears sole responsibility for the content of the data they provide, including responsibility for any infringement of third-party rights.
5. The User undertakes to review any changes made to these Terms.
6. Use of the form is voluntary. We will use the data solely to handle your enquiry. If you send photos or health-related information, you expressly consent to their processing for the purpose of consultation and organizing the process.

V. Liability of Flymedica Sp. z o.o.

1. Flymedica Sp. z o.o. is not liable for decisions made by the User based on information obtained as a result of using the information received after completing Flymedica's contact form.
2. Flymedica Sp. z o.o. states that all information obtained by the User through the Contact Form:
   1. is provided for informational purposes only,
   2. may not form the basis of any claims against Flymedica.
3. Flymedica Sp. z o.o. is not liable for the unavailability of the contact form resulting from causes beyond Flymedica's control, or from other technical causes related to the operation of the IT system and the website www.flymedica.pl.
4. Flymedica Sp. z o.o. reserves the right to amend the provisions of these Terms at any time. Amendments take effect as soon as the amended Terms are published on the website www.flymedica.pl.
5. Flymedica Sp. z o.o. reserves the right to discontinue this service entirely at any time, without giving reasons.

VI. Processing of Personal Data

In Flymedica forms and questionnaires, when age is needed for an initial consultation, we ask for age rather than a full date of birth. We do not ask for a passport or identity-card number. Please do not enter these details in free-text fields or attach photos of identity documents.
