§ 1 General Provisions
Flymedica Sp. z o.o., ul. Dąbrowskiego 77a, 60-529 Poznań, REGON 364674825, NIP 7792443359, KRS 0000622685, acts as the official representative of Ibrahim Koc Hair, address: Halide Edip Adıvar, Boru Çiçeği Sokağı No:23/A, 34382 Şişli/İstanbul, Türkiye — which is fully and solely responsible for the medical procedures performed at the patient's request.
Flymedica Sp. z o.o. is a VAT payer in Poland.
Flymedica Sp. z o.o. carries out the entire formal procedure on behalf of and for the benefit of the aforementioned Ibrahim Koc Hair in Istanbul referred to in paragraph 1, within the territory of the Republic of Poland, from the moment clients — patients — are acquired for the above-mentioned entities, until the finalization of the procedure at the above-mentioned clinics, including in particular:
- Compiling and handling patient applications.
- Collecting deposits from patients on behalf of and for the benefit of the above-mentioned entities specified in §3 (also referred to in these Terms as the clinics).
- Translating and forwarding the documents necessary for consultations regarding the procedures chosen by patients.
- Booking procedure dates at the clinics.
- Arranging interpreters on-site at the clinic.
- Arranging transfers (including airport transfers) and hotel accommodation.
- Overseeing the course of patients' stay in Turkey.
- Providing trichological advice.
- Caring for patients after their return to Poland, until the full results of the procedure are achieved.
§ 2 Booking of Procedures
Booking a procedure date at the clinic takes place after consultation, on the basis of the purchased flight tickets sent by the patient, and after payment of the deposit.
After selecting an available departure and procedure date, the patient receives by email confirmation of a temporary booking valid for 48 hours, instructions for purchasing flight tickets, and a link for online payment in PLN or bank transfer details in EUR / GBP.
If the patient does not pay the required deposit and does not purchase flight tickets within the specified time, the booking is cancelled, and Flymedica Sp. z o.o. bears no liability whatsoever in this regard.
On the basis of the flight tickets sent by the patient and the deposit paid, the booking becomes permanent and binding. Flymedica Sp. z o.o. sends by email all necessary information regarding travel, the procedure, and post-procedure recommendations.
§ 3 Payment Terms
The deposit for hair / beard / eyebrow transplant procedures at Ibrahim Koc Hair under the FUE, DHI, or Long DHI package is EUR 1390.24 net + 23% VAT, giving a total of EUR 1710.00 gross, converted into Polish złoty (PLN) at the exchange rate of 1 EUR = 4.30 PLN, which corresponds to PLN 5978.05 net + 23% VAT, giving a total of PLN 7353.00 gross.
During the promotional period, the deposit for hair / beard / eyebrow transplant procedures at Ibrahim Koc Hair under the FUE, DHI, or Long DHI package is EUR 951.22 net + 23% VAT, giving a total of EUR 1170.00 gross, converted into Polish złoty (PLN) at the exchange rate of 1 EUR = 4.30 PLN, which corresponds to PLN 4090.24 net + 23% VAT, giving a total of PLN 5031.00 gross.
The deposit is credited toward the organization of the procedure package and reduces the amount the patient pays on-site at the clinic, exclusively in cash in EUR.
§ 4 Rescheduling, Cancellation, Flight Cancellation, and Costs of Additional Services
If the patient cancels a previously booked procedure after the booking has been confirmed as permanent and binding, but before departure to Istanbul, the deposit paid is non-refundable and is used to cover the administrative and organizational costs associated with the planned procedure.
If the departure and procedure date is changed at the patient's request after the earlier booking has been confirmed as permanent and binding, the deposit paid is likewise non-refundable and is used to cover the administrative and organizational costs associated with the planned procedure.
If the flight is cancelled or delayed due to the fault of the airline, the deposit is not forfeited and is credited toward a new, agreed procedure date.
Rescheduling the procedure date is possible only once, and only if requested at least 30 days before the originally scheduled date. The deposit may be transferred to a new date, which must fall within 3 months of the original procedure date. If, in connection with the rescheduling, the price of the procedure changes, the patient is obliged to pay any resulting difference before the new date is confirmed.
If the patient requests a change of date less than 30 days before the procedure, the deposit is forfeited, and a new deposit must be paid in order to arrange a subsequent date.
The patient's failure to appear for the scheduled procedure, or failure to provide sufficient notice, results in the forfeiture of the deposit.
If a patient who is already on-site at the Ibrahim Koc Hair clinic cancels the procedure, this entails the obligation to cover the additional costs of the medical team's readiness in the amount of EUR 950 gross, as well as the costs of any additional hotel nights (beyond the standard 2-night stay) or other additional services agreed with the patient (if applicable).
Fees related to the medical team's readiness, refusal to perform the procedure, or additional medical requirements arise from the clinic's own regulations and are payable directly to the clinic in accordance with its rules.
Each additional hotel night in the hotel located in the clinic building, in a single room with breakfast included, costs EUR 60 gross.
Each additional hotel night in the hotel located in the clinic building, in a double room with breakfasts included, costs EUR 80 gross.
The surcharge for an accompanying person for the same transfers and accommodation in the same hotel room, in the hotel located in the clinic building, for 2 nights with breakfasts included, costs EUR 40 gross.
The surcharge for transfers between the clinic and the Marriott Sisli hotel is EUR 75 gross.
The surcharge for transfers between the clinic and another hotel chosen by the patient in the Beyoglu / Taksim area is EUR 100 gross.
§ 5 Medical and Organizational Conditions
The patient is obliged to promptly inform Flymedica of any change in their state of health or the introduction of new medications that were not included in the medical questionnaire submitted for consultation, even if the change occurs after the questionnaire was completed but before the procedure date. Such changes must be discussed with the anesthesiologist before the procedure, in order to rule out any contraindications and to allow the medical team to prepare appropriately for any changes in the medications administered.
If a patient who has already made a permanent and binding booking reports new medications or treatment which, in the decision of the clinic's anesthesiologist — also based on relevant test results — rules out performing the procedure on that patient, the procedure is cancelled, and the deposit paid is either transferred to another date, if the refusal occurs more than 15 business days before the planned departure date, or is non-refundable and used to cover the administrative and organizational costs associated with the planned procedure, if the notification occurs less than 15 business days before the planned departure date.
Concealing medical information or failing to disclose it may result in the clinic refusing to perform the procedure for reasons of the patient's health and life safety, and will result in the obligation to cover the full remaining costs of the booked procedure and any additional services (if applicable); the deposit paid is non-refundable and is used to cover the administrative and organizational costs associated with the planned procedure.
For patients with HIV infection, an additional fee of EUR 1250 gross applies, payable on-site at the clinic in cash in EUR.
Ibrahim Koc Hair reserves the right to refuse to perform the procedure if the patient arrives for the consultation at the clinic or for the procedure under the influence of alcohol, drugs, or other psychoactive substances. In such a case, the patient will be charged the full cost of the procedure and any additional services ordered (if applicable), and the deposit paid is non-refundable and is used to cover the administrative and organizational costs associated with the planned procedure.
Ibrahim Koc Hair reserves the right to refuse to perform the procedure for reasons of the patient's health and life safety, if the patient's state of health on-site at the clinic does not allow the procedure to be carried out. In such a case, the patient is obliged to cover, on-site at the clinic, the remaining costs of the medical team's readiness in the amount of EUR 650 gross, and the deposit paid is non-refundable and is used to cover the administrative and organizational costs associated with the planned procedure.
Ibrahim Koc Hair does not perform procedures on patients with Hepatitis C. If a Hepatitis C infection is detected on-site at the clinic, the patient is obliged to cover the remaining costs of the medical team's readiness in the amount of EUR 650 gross, payable to Ibrahim Koc Hair, and the deposit paid is non-refundable and is used to cover the administrative and organizational costs associated with the planned procedure.
Flymedica bears no liability for medical decisions made by the clinic.
§ 6 Complaint Handling Procedure
Complaints regarding the services provided by Flymedica may be submitted by email to kontakt@flymedica.pl or in writing to the company's registered office address.
Each complaint will be handled within 14 business days from the date it is received by Flymedica Sp. z o.o.
The customer will be informed of the outcome of the complaint electronically (to the email address provided) or in writing.
Complaints concerning the course or outcome of a medical procedure will be forwarded by Flymedica, on the patient's behalf, directly to the clinic that performed the procedure.
§ 7 Right of Withdrawal from the Contract
In the case of procedure-booking and stay-organization services, withdrawal from the contract is not possible after the booking has been made and the deposit paid, due to the specific nature of the services provided and the related organizational commitments.
The above conditions form an integral part of the offer and the basis for the organization and performance of procedures. In the event of price list changes, the price applicable to the patient is the price in effect on the date the booking was made.
§ 8 Liability
Flymedica is liable solely for the proper performance of its intermediary and trip-organization services.
Flymedica bears no liability for the course, manner of performance, or outcome of the medical procedure carried out by Ibrahim Koc Hair.
§ 9 Final Provisions
Flymedica Sp. z o.o. reserves the right to amend these Terms. Any changes will take effect from the date they are published on the Flymedica Sp. z o.o. website.
Any disputes arising from the performance of these Terms will be resolved in accordance with applicable Polish law.
